Here at YOURhealth we are part of the Community Mental Health Transformation programme where we are supporting positive changes to how we as a trust deliver Community Mental Health Services.

We want to ensure all who require support with their wellbeing are able to have a voice and be involved in their care and understand what support is available and what you can do to also look after your own wellbeing.

The role of the mental health and wellbeing coach is a ‘New Look’ hybrid role that takes elements of the Healthtrainer role and Community Link, Social Prescribing role to address and focus on your mental health and wellbeing needs.

“Support has been invaluable and has helped me see a light at the end of the tunnel. Rebecca is an asset to the team”
